Application for Library Card


You must be an Ohio resident to register for a library card. After completing the application, you need to present in person photo identification with your name and current address, such as a driver’s license or Ohio state ID. Photo ID without the current address must be accompanied by printed personal mail postmarked within four weeks.


A parent or guardian must sign for a minor (under age 18) and present photo identification that has the parent’s or guardian’s name and current address, such as a driver’s license or Ohio state ID. Photo ID without the current address must be accompanied by personal mail postmarked within four weeks. The child applicant must be present.
